ALL Students MUST use this ONLINE form to renew your enrollment in classes at WAG. If you have multiple dogs you will need to submit forms for each dog individually. Paper forms have been eliminated.
You will receive an email copy of this form to the address you included which will confirm receipt of your request to continue in classes. Please note your options to follow up with payment for the class. Upon receipt of payment and confirmation of space availability you will be re-enrolled. Thanks!

If you are unsure on the level you will be in - mark unsure - and your trainer or the agility coordinator will confirm with you when it is determined. When the form is complete - click on the Send button above to submit your form. If you are paying ONLINE then click on the PayPal button below to pay.
