Fill in your information above and mark your level for your dog that you wish to enroll. Pre-Agility students may only enroll by mail for their first time. If you are unsure on the level you will be in - mark unsure - and your trainer or the agility coordinator will confirm with you when it is determined. When complete - click on the Send button above to submit and then click on the PayPal button below to pay.
You must SEND in the above form for each dog you wish to have included in classes. Do NOT just send in payment without the form or the form without the ONLINE payment. Failure to do so may keep you from being added to the class Rosters. Thanks!
